If you’re ready to make an appointment:

Call your insurance company, ask for “outpatient mental health benefits” and they should tell you what you need to do thenQuestions to ask specifically: What is my Co-Pay? Co-insurance? Do I have a deductible and have I met it/how much is it?Is Authorization required? Are there any excluded mental health benefits (i.e. family, couple therapy)? Are there any excluded diagnoses (i.e. ADHD)? Are 90781 (assessment) and 90834 (45 minute individual sessions) 90837 (60 minute individual session )and 90847 (family sessions) covered? Some companys give you a special number that you in turn give to me. Some will have you call them back & give them the time and date of your first appointment.  I am on most insurance panels: Aetna, BCBS, Health Advocate EAP, Magellan, Mines & Associate’s EAP; St. Luke’s EAP, St. Barnabas EAP, & United Health Care Value Options.

If you don’t have insurance, call me to set up an appointment.  The first session (1 hour, 15 minutes) is $160; 1 hour individual is $145.  45-50 minute sessions are $110 for an individual; $140 for a couple.  You can pay by check, cash, or credit card, using Paypal, from your own computer.  Just go to  “Client Forms” page.
